Currently, targeted and immunotherapy contributes to the treatment of malignant neoplasms. One of the examples is the treatment of skin melanoma.</p> <p><bold><italic>AIMS</italic></bold><italic>: </italic>This study aimed to investigate the influence of therapeutic tactics on epidemiological indicators in the morbidity of the skin melanoma.</p> <p><bold><italic>MATERIALS AND METHODS</italic></bold><italic>:</italic> The study used data of the Krasnoyarsk territory cancer-register.</p> <p><bold><italic>RESULTS</italic></bold><italic>:</italic> In the Krasnoyarsk territory from 2016 to 2020 registered 65,042 cases of malignant neoplasms, of which 30,532 cases (46.9%) reported among men and 34,510 cases (53.1%) among women. In the same period, registered 899 cases of skin melanoma, 343 cases among men (38.2%) and 556 cases among women (61.8%). The most important stage in the treatment of skin melanoma is the disease stage, because only favorable prognosis characteristic for early stages of the disease.</p> <p><bold><italic>CONCLUSIONS</italic></bold><italic>:</italic> Routine using modern antitumor drug therapy in everyday clinical practice has significantly changed the results of skin melanoma treatment.</p></abstract><trans-abstract xml:lang="ru"><p><bold><italic>Обоснование</italic></bold><italic>.</italic> Одна из наиболее актуальных задач современной онкологии – разработка и внедрение новых методов диагностики и лечения злокачественных новообразований, повышение их эффективности и безопасности. В настоящее время большой вклад в лечение данной патологии вносят иммуно- и таргетная терапия, и один из наиболее ярких примеров – лечение меланомы кожи.</p> <p><bold><italic>Цель</italic></bold><italic>. </italic>Изучить влияние лечебной тактики на эпидемиологические показатели при заболеваемости меланомой кожи на примере Красноярского края.</p> <p><bold><italic>Материалы и методы</italic></bold><italic>.</italic> Использованы данные канцер-регистра Красноярского края.</p> <p><bold><italic>Результаты</italic></bold><italic>. </italic>С 2016 по 2020 г. в Красноярском крае зарегистрировано 65 042 впервые установленных случая злокачественных новообразований, из них 30 532 (46,9%) – у мужчин, 34 510 (53,1%) – у женщин. За этот же период выявлено 899 случаев меланомы кожи: 343 (38,2%) – у мужчин, 556 (61,8%) – у женщин. Наибольшее значение в лечении меланомы кожи имеет стадия заболевания: благоприятный прогноз характерен только для ранних стадий.</p> <p>Применение в Красноярском крае схемы «дабрафениб + траметиниб» увеличилось с 6 (2017 г.) до 88 случаев лечения (2020 г.), а по итогам 2021 г. зафиксировано уже 164 таких случая. Также активно в регионе используется схема «вемурафениб + кобиметиниб», которая впервые применена в 2018 г. – в 8, в 2020 г. – в 55, а в 2021 г. – уже в 140 случаях лечения. Рост доли иммуноонкологической терапии в лечении пациентов с меланомой кожи ещё более показателен. Впервые в Красноярском регионе комбинация «ниволумаб + ипилимумаб» была применена в 2018 г. – в 5 случаях лечения, тогда как в 2020 г. – уже в 54. Применение пембролизумаба в лечении пациентов с меланомой кожи увеличилось с 6 случаев в 2017 г. до 459 в 2020 г.</p> <p>Анализ стандартизованного показателя смертности выявил снижение на 14,82% смертности от меланомы кожи (оба пола) за период 2016–2020 гг. в Красноярском крае: с 1,35±0,35 (2016 г.) до 1,15±0,31 на 100 тыс. населения (2020 г.).</p> <p><bold><italic>Заключение</italic></bold><italic>. </italic>Внедрение в повседневную практику современной противоопухолевой лекарственной терапии существенным образом улучшило результаты лечения меланомы кожи.</p></trans-abstract><kwd-group xml:lang="en"><kwd>skin melanoma</kwd><kwd>targeted therapy</kwd><kwd>immunotherapy</kwd><kwd>Krasnoyarsk region</kwd><kwd>epidemiology</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>меланома кожи</kwd><kwd>таргетная терапия</kwd><kwd>иммунотерапия</kwd><kwd>Красноярский край</kwd><kwd>эпидемиология</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">State report «On the state of sanitary and epidemiological well-being of the population in the Russian Federation in 2020».
