In these groups, the tumor removal operation in patients ended with plastic tissue defect and the group comparisons (n = 155) (after removal of the tumor, simple linear wound closure was performed).</italic></p> <p><bold>Results. </bold><italic>I</italic><italic>t was found that pronounced lymphoid tumor infiltration in patients with primary skin melanoma as a predictor of a favorable prognosis (in terms of the occurrence of locoregional recurrence) is realized in patients with plastic defect replacement significantly 2 times more often than in patients without plastic surgery in the period from 12 to 60 months of observation.</italic></p> <p><bold>Discussion. </bold><italic>T</italic><italic>he dependence of the occurrence of locoregional relapses in patients on lymphoid infiltration of the tumor and the performance of plastic surgery was revealed. In general, all patients who underwent plastic surgery have an advantage in terms of the occurrence of locoregional relapses in the long term for a period of up to 5 years by 12.5%. In patients with severe lymphoid infiltration and plastic surgery, locoregional relapses occur almost 2 times less often than in patients without plastic surgery, starting from a follow-up period of 12–36 months by 20.6% (22.9% and 43.5%, respectively; p = 0.008), and in the period from 36 to 60 months of observation by 24.7% (25.3% and 50.0%, respectively; p = 0.002).</italic></p> <p><bold>Conclusion. </bold><italic>T</italic><italic>he use of plastic techniques for closing a wound defect in patients with skin melanoma with pronounced lymphoid tumor infiltration reduces the risk of gross scarring and halves the risk of locoregional metastasis as compared to linear suturing of the postoperative defect.</italic></p></abstract><trans-abstract xml:lang="ru"><p><italic>Целью исследования было</italic> <italic>проанализировать влияние пластических способов закрытия дефекта после иссечения первичной меланомы кожи в зависимости от лимфоидной инфильтрации опухоли.</italic></p> <p><bold>Материал и методы. </bold><italic>Исследовались пациенты с первичной меланомой кожи, пролеченные в 2013 г. (n = 337), которые были рандомизированы на 2 группы методом слепого отбора: на основную (n = 182), в которой у пациентов операция удаления опухоли заканчивалась пластикой дефекта тканей, и группу сравнения (n = 155) – после удаления опухоли выполнялось простое линейное ушивание раны.</italic></p> <p><bold>Результаты. </bold><italic>У</italic><italic>становлено, что выраженная лимфоидная инфильтрация опухоли у пациентов с первичной меланомой кожи как предиктор благоприятного прогноза (в плане возникновения локорегионарного рецидива) реализуется у пациентов с пластическим замещением дефекта достоверно в 2 раза чаще, чем у пациентов без пластики в период с 12–60 мес наблюдения.</italic></p> <p><bold>Обсуждение. </bold><italic>Выявлена зависимость возникновения локорегионарных рецидивов у пациентов от лимфоидной инфильтрации опухоли и факта выполнения пластики. В целом все пациенты с пластикой имеют преимущество в плане возникновения локорегионарных рецидивов в отдалëнной перспективе за период до 5 лет на 12,5%. У пациентов с выраженной лимфоидной инфильтрацией и пластикой почти в 2 раза реже возникают локорегионарные рецидивы, чем у пациентов без пластики начиная с периода наблюдения от 12–36 мес – на 20,6% (22,9 и 43,5% соответственно, р = 0,008), а в период от 36–60 мес наблюдения – на 24,7% (25,3 и 50,0% соответственно, р = 0,002).</italic></p> <p><bold>Заключение. </bold><italic>Использование пластических приëмов закрытия раневого дефекта у пациентов с меланомой кожи с выраженной лимфоидной инфильтрацией опухоли снижает риск грубого рубцевания и в 2 раза снижает риск локорегионарного метастазирования по сравнению с линейным ушиванием послеоперационного дефекта.</italic></p></trans-abstract><kwd-group xml:lang="en"><kwd>skin melanoma</kwd><kwd>lymphoid tumor infiltration</kwd><kwd>surgical treatment</kwd><kwd>plastic defect replacement</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>меланома кожи</kwd><kwd>лимфоидная инфильтрация опухоли</kwd><kwd>хирургическое лечение</kwd><kwd>пластическое замещение дефекта</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Burton A.L., Roach B.A., Mays M.P., Chen A.F., Ginter A.R.B., Vierling M.A., et al.
