The medical approach, in which liver resection is performed as the first stage of treatment is called liver-first. The rationality of this approach is justified by elimination of the most prognostically significant tumor foci, which are hepatic metastases, with subsequent treatment of primary colorectal tumor.</p> <p><bold>AIM:</bold> To evaluate the immediate and long-term treatment results in patients with metastatic colorectal cancer, focused on surgical removal of synchronous liver metastatic foci as the first stage of treatment.</p> <p><bold>MATERIALS AND METHODS:</bold> A single-center, observational, prospective cohort study was carried out from February 2017 to June 2023 in the abdominal surgical department of the P.A. Herzen Moscow State Medical Institute — a branch of the National Medical Research Radiological Center (Moscow, Russia).</p> <p><bold>RESULTS:</bold><bold> </bold>The study included 70 patients (37 men and 33 women). The treatment plan, according to the previously approved protocol, was carried out in 25 patients out of 70 (35.7%). Mean duration of the first stage was 235 min (125–328 min), mean blood loss volume 450 ml (100 ml–1300 ml). Mean hospitalization length was 17 bed days (12–21). The 30-day mortality rate was 12.8% (5 out of 39 patients). Overall, 1-year and 3-year survival rate after second stage of the treatment was 78% and 52%, respectively. At the same time, 1-year progression-free survival rate was 25%, and 3-year survival rate was 11%.</p> <p><bold>CONCLUSION:</bold> In rare clinical situations, when synchronous bilobar liver metastases are accompanied by an asymptomatic primary tumor, the best choice is to follow the liver-first protocol. However, it should be noted that not every patient at stage IV colorectal cancer can be a candidate for combined two-stage treatment, which is determined by somatic status and high risk of disease progression.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> На момент первичного выявления колоректального рака после дообследования синхронное метастатическое поражение печени определяется у 20–25% больных. Подход, при котором первым этапом выполняется резекция печени, в англоязычной литературе носит название «liver-first approach». Рациональность данного подхода обосновывается эрадикацией наиболее прогностически значимого опухолевого очага, соответствующего печёночному метастазу, с возможностью последующего лечения первичной колоректальной опухоли.</p> <p><bold>Цель </bold>— оценить непосредственные и отдалённые результаты лечения больных с метастатическим колоректальным раком при использовании подхода, ориентированного на хирургическое лечение синхронных метастатических очагов в печени первым этапом.</p> <p><bold>Материалы и методы.</bold> Исследование было выполнено в дизайне одноцентрового обсервационного проспективного когортного исследования в период с февраля 2017 по июнь 2023 года на базе абдоминального хирургического отделения Московского научно-исследовательского онкологического института имени П.А. Герцена — филиала Национального медицинского исследовательского центра радиологии. В исследование было включено 70 больных (37 мужчин и 33 женщины).</p> <p><bold>Результаты.</bold> План лечения, согласно ранее утверждённому протоколу, был выполнен у 25 больных из 70, что составило 35,7%. Средняя продолжительность первого этапа оперативного вмешательства составила 235 минут (от 125 до 328 минут), средний объём кровопотери — 450 мл (от 100 мл до 1300 мл). Среднее время пребывания в стационаре составило 17 койко-дней (от 12 до 21 койко-дня). Уровень 30-дневной летальности составил 12,8% (5 из 39). Общая выживаемость в сроки 1 года и 3 лет после окончания второго этапа хирургического лечения составила 78% и 52% соответственно. При этом 1-летняя выживаемость без прогрессирования составила 25%, а 3-летняя — 11%.</p> <p><bold>Заключение.</bold> В редких клинических ситуациях, когда синхронное билобарное метастатическое поражение печени сочетается с бессимптомной опухолью, наилучшим выбором будет следование протоколу liver-first.
