It is particularly difficult to detect when enlarged regional metastatic lymph nodes are located virtually on the border with the primary tumor localization. In tumors of the salivary glands, even modern capabilities of radiological and morphological diagnostics do not always allow clear differentiation of organ specificity, categories of benignity or malignancy , as well as distinguishing a primary tumor from a metastatic process.</p> <p><bold>Case description:</bold> In a patient with obvious clinical signs of nodular thyroid gland lesion after initial referral to the outpatient clinic, a malignant process was cytologically excluded. However, during additional examination, tumor pathology of the upper third of the neck was detected, which did not confirm its organ specificity to the major salivary glands and regional lymph nodes according to combined data from modern radiological imaging methods. The results of cytological diagnostics of several tumor-like formations on the neck contributed to radical treatment in the setting of a head and neck surgical oncology department, with planned histological and immunohistochemical investigation. The morphologically confirmed malignant origin from salivary gland tissue without organ specificity yielded varying histological reports. In the absence of disease recurrence , given contradictory morphological findings, a review of histological slides and immunohistochemical investigation of the tumor was conducted at a reference center. The review confirmed the presence of a pleomorphic adenoma of aberrant salivary glands in the variant of retrograde differential diagnosis.</p> <p><bold>Conclusion: </bold>A clinical observation is presented in which only the results of a control immunohistochemical investigation allowed formulation of a final clinicopathological diagnosis of pleomorphic adenoma of the ectopic salivary gland with a multicentric growth pattern in the variant of retrograde differential diagnosis with different forms of carcinoma.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Дифференциальная диагностика злокачественных новообразований в области шеи представляет значительные трудности с точки зрения обнаружения первичной органной опухоли и внеорганного первичного поражения. Особенно сложно определить опухоль при наличии увеличенных регионарных метастатических лимфоузлов, расположенных на границе с первичной локализацией опухоли. При опухолях слюнных желёз даже современные возможности лучевой и морфологической диагностики не всегда позволяют чётко дифференцировать органоспецифичность, категории доброкачественности или злокачественности, а также отличить первичную опухоль от метастатического процесса.</p> <p><bold>Описание клинического случая.</bold> У пациентки с явными клиническими признаками узлового поражения щитовидной железы после первичного обращения в поликлинику цитологически был исключён злокачественный процесс. Однако при дополнительном обследовании выявлена опухолевая патология верхней трети шеи, которая не подтвердила её органоспецифичность к большим слюнным железам и регионарным лимфатическим узлам по комбинированным данным современных методов лучевой диагностики. Результаты цитологической диагностики нескольких опухолевидных новообразований на шее помогли провести лечение в условиях хирургического отделения опухолей головы и шеи с плановым гистологическим и иммуногистохимическим исследованием. Морфологически подтверждённый злокачественный генез из ткани слюнной железы без органоспецифичности показал разные гистологические заключения. При отсутствии рецидива заболевания, с учётом противоречивых морфологических заключений, в стационаре провели пересмотр гистологических сканов иммуногистохимического исследования опухоли в референс-центре. Пересмотр подтвердил наличие плеоморфной аденомы аберрантных слюнных желёз в варианте ретроградной диагностики.</p> <p><bold>Заключение.</bold> Представлено клиническое наблюдение, в котором только результаты контрольного иммуногистохимического исследования позволили сформулировать окончательный клинико-патологический диагноз плеоморфоной аденомы эктопической слюнной железы с мультицентрической формой роста в варианте ретроградной дифференциальной диагностики с разными формами карциномы.</p></trans-abstract><kwd-group xml:lang="en"><kwd>pleomorphic adenoma</kwd><kwd>salivary gland</kwd><kwd>extraorganic tumor</kwd><kwd>myoepithelial carcinoma</kwd><kwd>cylindroma</kwd><kwd>lymph nodes</kwd><kwd>differential diagnosis</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>Плеоморфная аденома</kwd><kwd>слюнная железа</kwd><kwd>внеорганная опухоль</kwd><kwd>миоэпителиальная карцинома</kwd><kwd>цилиндрома</kwd><kwd>лимфатические узлы</kwd><kwd>дифференциальный диагноз</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Kostyaeva MG, Eremina IZ, Kastyro IV.
