THE INFORMATIVENESS AND MEDICAL ACTIVITY OF MEDICAL WORKERS CONCERNING ISSUES OF HCV/HBV PREVENTION AND DISPENSARY OBSERVATION OF A-HCV/HBSAG-POSITIVE PATIENTS

Abstract

The article presents the results of questionnaire survey of physicians concerning issues of prevention and dispensary observation of а-HCV/HBsAg positive patients. The study established inadequate awareness and medical activity of physicians concerning issues of prevention and dispensary observation of patients from risk group and patients with viral hepatitis C/B. The working experience and medical length of service without any additional training give no guarantees of sufficient knowledge in issues of prevention, timely detection and corresponding observation of a-HCV/HBsAg positive patients.

THE EPIDEMIOLOGICAL CHARACTERISTIC OF DISEASES OF MUSCULOSKELETAL SYSTEM

Abstract

The purpose of the study. To analyze prevalence of diseases of musculoskeletal system in the Arkhangelsk region. The methods. The data of official statistics was used to implement analysis of general morbidity of main diseases of musculoskeletal system of adult population the Arkhangelsk region during 2010-2015 as compared with corresponding morbidity in the North-Western Federal and the Russian Federation in general. The results. In the Arkhangelsk region, among general morbidity percentage of diseases of musculoskeletal system made up to 4.2% (7th rank). The increasing of common morbidity of diseases of musculoskeletal system in adult population was marked in the Arkhangelsk region up to 14% and in the Russian Federation up to 7%. Since 2010, in the Arkhangelsk region the study established increasing of common morbidity rheumatoid joint inflammation up to 22%, spondylopathy up to 65%, osteoarthrosis up to 32.4%, osteoporosis up to 46% and systemic diseases of connective tissue up to 11.5% surpassing average Russian indices. At that, number of cases of reactive arthritis decreased up to 17.2% Conclusions. In the Arkhangelsk region, the register of diseases of musculoskeletal system on the basis of region rheumatological center is to be developed to implement an objective assessment in the existent situation.

THE CLINICAL ECONOMIC ANALYSIS OF SCHEMES OF GRADED ANTI-BACTERIAL THERAPY AS A LEVEL OF AMELIORATION OF QUALITY OF MEDICAL CARE SUPPORT OF PATIENTS WITH UROLOGICAL PATHOLOGY

Abstract

The necessity of implementing clinical economic studies was conditioned by a significant prevalence of infections of urinary tracts, wide variety and high cost of antibacterial pharmaceuticals for their treatment. The sampling included 539 cases of application of step schemes of antibiotics treatment in patients of urological departments in 2014-2015. Overall, six the most prevalent schemes (87.4% of all cases) were picked out. Every scheme was analyzed by application rate, clinical efficiency, treatment cost and average duration of patient stay in hospital. The most clinical efficiency was established for the scheme that included amoxicillin + сlavulanic acid in injection and peroral components without implementing peri-operational antibiotics prevention (97.7%). However, period of hospitalization was in average 2.9 days longer and transition of patient from injection component of therapy to a peroral one occurred 2.3 days later than in case of application of the same pharmaceutical in combination with peri-operational antibiotics prevention (efficiency index 96.0%). It is established that the scheme with amikacin being from economic point of view the most attractive one (cost of treatment is 2.5-6.1 times lower and period of hospitalization the shortest) and differing by high clinical efficiency (95.2%) is applied only in 4.4% of analyzed cases because amikacin has many side effects restricting its application in medical practice. The received information permits physician to rationally prescribe optimal schemes of treatment that decreases costs of therapy, risk of development of complications initiated by antibacterial therapy and also increases quality of life of patient.

THE EVALUATION OF SOCIAL EFFECTIVENESS OF PRIMARY MEDICAL SANITARY CARE

Abstract

Social efficiency of the health care system is a priority type of sector effectiveness at every management level. The actual transformations in organization of medical care require investigation of effectiveness and should not adversely effect on indices of medical organizations functioning. The satisfaction of the population with quality and accessibility of medical care are related to social efficiency evaluation. The sociological surveys pay a special attention to studying conditions of medical services provision, which are assessed by patients subjectively. The article presents the results of a study of patients concerning satisfaction with primary medical sanitary care before and after organization of the Clinical Medical Center in Chita. The estimation of efficiency was implemented on the basis of factor conditionality of satisfaction level. Identification of degree of influence of each factor was implemented using calculation of relative and absolute risks. At that, outcome of medical care provision in the form of satisfaction was considered as an effective criterion and conditions of medical care provision as "risk factors". The study results established increasing of index of patient satisfaction with medical care in the Chita Clinical Medical Center, that testifies high social efficiency. The influence of such factors conditioning a decrease of degree of satisfaction as inconvenient working schedule of physician and violation of ethical deontological standards by the medical workers in relationship with patients was established. The results of studying factor conditionality of patient satisfaction allow to consider them in development of management measures. The necessary elements of development of the system of primary medical sanitary care include implementation of modern information systems, a uniform electronic medical record, remote appointment to physician visit, telemedicine technology, systems of electronic document management.

ABOUT LEGAL FUNCTIONAL LITERACY OF POPULATION IN SPHERE OF IMPLEMENTATION OF RIGHTS OF THE MINORS IN THE AREA OF HEALTH CARE

Abstract

Nowadays, the crisis of family as a fundamental social institute is one of the serious problems of the Russian society. At the actual stage of national development, the given occurrence is aggravated by atmosphere of total protracted social economic crisis related to period of reforms. The progressing deterioration of condition of family in all aspects of its life activities is a generalized process negatively impacting the development and education of children. The juridical competence based on legal functional literacy is one of the determinants conditioning behavior of parents as related to the minors. The purpose of the study is to evaluate the level of legal functional literacy of population in issues of implementation of rights of the minors in the area of health protection. The results of sociological survey permitted to conclude that the degree of legal awareness of population at the level of administrative municipal formation of the subject of the Russian Federation in issues of legal status of the minors is inadequate. Such stratification characteristics of citizen as family status, social status, level of material well-being came as predictors conditioning existing situation. The established predictors of legal awareness of citizen such as social status, family status, level of material well-being, permitted to establish a particular category of population - a target risk group untenable within the framework of implementation of rights of the minors, including health protection due to inadequate level of legal competence. The determinants presented in the established cluster of population are relatively manageable and they can be compensated by rationale and efficient policy of the state authorities and health care system in issues of increasing level of life and legal education of population including the field of health protection.

THE INDIVIDUAL STYLE OF DECISION MAKING BY ADMINISTRATORS OF MEDICAL ORGANIZATIONS IN PERSPECTIVE OF EVALUATION OF MANAGEMENT POTENTIAL

Abstract

In assessing management potential of a manager, it is necessary to consider one's personal characteristics, in particular, management decision-making style. A physician repeatedly makes clinical decisions on patients’ treatment and this experience shapes his/her individual decision-making style, which in turn defines personal features of physician as a leader. Purpose of study. To evaluate the management potential of leaders of medical institutions of the Tomsk oblast on the basis of investigation on their decision-making styles. The decision-making style was identified using A. Rowe's technique (Decision Style Inventory) widely applied in various research areas. The sampling of survey included 1097 physicians, including head physicians (4,8%), deputy head physicians (10,1%), heads of department (11,9%). Decision-making styles of leaders differ depending of the various levels of management. The analytical style dominates among head physicians, followed by conceptual style. For the largest cohort of deputy head physicians analytical style also dominates, but the second rank of dominating styles is for directive style. In general, moving down administrative staircase the percentage of managers with dominating conceptual style is decreasing and percentage of those who are oriented on individual (authoritarian) decision-making processes and also those who are focused more on human relationship than on tasks solution increases. The possibilities of transformation of individual style of decision making are limited and require conscious efforts, that tasks a complicated problem before leaders of medical institutions concerning assessment of management potential of development and training of long-term human resources reserve.

THE ANALYSIS OF FUNCTIONING OF MEDICAL COMMISSION OF THE LARGE HOSPITAL POLYCLINIC ASSOCIATION RELATED TO PROLONGATION OF MEDICAL CERTIFICATES

Abstract

The feature of expertise of temporary disability is that expert decisions are considered according legislation as a basis of providing patients with various types of social protection and, in particular, exemption from work during acute period of disease. Since this is an issue of spending budget funds and extra-budgetary funds for these activities, all the procedures of expertise of temporary disability are regulated by legislative and regulatory acts that not always clearly define actions of doctors in various situations, and sometimes do not correspond to constantly changing conditions in the system of medical organizations functioning. The recent alterations in the legislative base led to a significant increase of burden on medical commission of the prolongation of medical certificates, which was already high enough due to appeals related to referral of patients to medical social expertise, medical examination of citizens, etc. The article presents the analysis of medical commission functioning of a large hospital-polyclinic association related to prolongation of medical certificates. The authors pursued the goal of identifying unfavorable trends in the medical commission functioning and identify ways to optimize it. The assessment of current status of the medical commission functioning demonstrated that prevention of injuries, especially domestic ones, as well as prevention of exacerbations of chronic diseases, including within the framework of dispensary observation of corresponding contingents of patients are of great importance for reducing the temporary disability of citizens.

THE THIRTY YEARS’ EXPERIENCE OF SURGICAL CARE TO FOREIGN PATIENTS IN STATE ORTHOPEDICS CLINIC

Abstract

The history of medical tourism counts several millennia. however, medical tourism became a wide-spread occurrence only to the end of XX century, in the age of globalization that favored drastic increasing of low budget mass tourism at the expense of development of Internet and accessibility of air flights. The purpose of study. To share experience of organization of planned medical care to foreign patients in conditions of state orthopedics clinic. During 1983-2016 in the Academician G.A. Ilizarov Russian Research Center «Restorative Traumatology and Orthopedics» 1304 foreign patients coming from 82 countries were treated. In the Ilizarov Center, to resolve all paramedical issues related to stay of foreign patients, an international department is functioning since July 14 1988, organized on the basis of a group accomplishing organization of scientific technical cooperation. The major tasks of the international department are organization and promotion of development of international relationships of the Center. The international department is a unique section having no analogues in other federal health care institutions. The experience of the Ilizarov Center in rendering orthopedic care to foreign citizen is a leading one for Russia. The main factor determining attractiveness of the Center to foreigners is a uniqueness of technology of Ilizarov transosseous osteosynthesis in combination with enormous experience in treatment of orthopedic pathology. Conclusions. The medical tourism becomes one of major sections of modern global economics. The amelioration of quality and accessibility of high-tech medical care under dedicated state policy, legislation, community and mass media support can at least to shorten withdrawal of funds from our country and support one's own centers of competence.
